  8- Wiping cloth and food debris in hand sink near dish machine. Corrected.
  12- 3-1qt. containers half and half with 3/8/09 expiration date. Voluntarily discarded and denatured.
  14-2 cutting boards at cooks line and cutting board at food prep sink with stains and grooves.
  20- Salad dressing at 58F. Voluntarily discarded and denatured.
  33- Food thawing in 3 basin sink. Corrected.
  35- Open dry goods stored in open original packaging on dry storage shelf.
  47- Food debris around dry storage shelving, walk in cooler and walk in freezer. Debris in closets next to mop sink.

  Facility aware of problem and has provided MCHD with PCO report and will continue to do so until problem is resolved.
