  Facility corrected most of the violations noted on the inspection report dated 5/11/15, prior to this re-inspection
  1. Facility lacking a MCHD CFSM. Facility has 30 days, 6/11/15, to register for an approved CFSM course or provide proof of certification.
  Christopher Carbonara will apply for a 2 yr extension on his present certification.
  Failure to correct and maintain any and all violations may lead to legal action, citations and or closure.

  53. the owner is in the process of replacing the damaged floor tiles - not all have been replaced or repaired
