  1: Current MCHD Certified Food Sanitation Manager (CFSM) certificate lacking at facility. Facility stated they had just changed managers and were
  awaiting new certificate. Post when received. EHS will verify that new certificate has been applied for. Application may be downloaded from
  www.health.montcopa.org.
  8: Ware washing room hand sink blocked by boxes stored in basin. Hand sinks must be kept clear and accessible at all times.
  11: Three half-gallon containers Hood egg nog found in True cooler with sell-by date of 11/27/12. Facility voluntarily discarded. COS
  42: Wet mop stored in mop sink basin. Hang or invert to dry.
  52: Dumster lid open at time of inspection. Keep closed at all times.
  53: Missing ceiling tiles around hot water heater. Replace.
  53: Chipping paint on back room floor. Provide a surface that is sealed, smooth, nonporous and easily cleanable. (Rx3)
  NOTE: Repeated violations that go uncorrected may be subject to further legal action being taken by MCHD.
