  49: Leak observed at pipe on air conditioning/heater unit in closet. Moisture accumulation observed on floor. Repair leak immediately.
  53: Damaged floor tiles observed throughout facility (Rx4) Facility has started replacing tiles. Continue until floor throughout facility is in good repair.
