OBSERVATIONS AND CORRECTIVE ACTIONS
  Item No.                                                 Violations cited in this report must be corrected within the time frames below.
  The followings were observed during pre-opening inspection today. Correct and maintain prior to opening inspection.
  -Provide all missing parts on foodservice equipment (ventilation system filters & front panels, soda machine, Menu board, etc).
  -Remove all construction tools and materials from the premise.
  -Remove all foods (soft serve mix, french fries) from the premise. No foods or beverages are to be stored on premise prior to final approval to operate.
  -Maintain all refrigeration at 41F or below and freezers at 0F or below ( UC refrig., walk-ins, etc).
  -Clean and maintain all equipment from dust and debris.
  -Clean and maintain all floors including walk-in refrigeration units. and below ice machine.
  -Live flies like insects observed. Maintain premise to be vermin free.
  -Provide all missing ceiling tiles.
  ** In order to prepare for final opening inspection for license approval , the Opening Inspection preparation List should be checked for
  completion prior to the inspection. Handout issues today.
  Licensee is to contact the Dept. at least 3 business days prior to arrangement for an opening inspection.
  If facility is not in compliance condition during final opening inspection, a fee of $133.00 must be paid to the Dept.
  prior to arrangement of second opening inspection.
